Customizing toolbar and menu with OXSE 6.18.2.0?

Hi all, I'm working on some GUI plugins and I would like to add some custom toolbar and menu within OXSE 6.18.2.0.
I've followed this tutorial http://oxpedia.org/wiki/index.php?title=Gui_Plugin_Development (section Customized new buttons in the panel) for versions up to 6.18.1, but with the latest release the API has changed quite a bit.

Is there an updated tutorial for 6.18.2?

The example should still work with 6.18.2. What exactly stopped working?
